Well Child Check-ups

Pediatricians consider preventive care to be a crucial part of a child's total health care. Well child check-ups offer an opportunity to evaluate and monitor your child's growth and development. These appointments are scheduled to allow the additional time that is required to assess and discuss your child, your family and your concerns.

Immunizations are discussed and given (if appropriate) at these visits. We follow the American Academy of Pediatrics Guidelines for the schedule of these well visits and immunizations. Vaccine Information Statements (VISs) are information sheets produced by the Centers for Disease Control and Prevention (CDC) www.cdc.gov/nip that explain both the benefits and risks of a vaccine. Below each age group is a list of the recommended immunizations.

We also offer "prenatal visits" for expectant parents. This "get acquainted" appointment gives you an opportunity to meet the doctor and tour the office.

Parents will need to complete an information form at each check up.  This can be done in the office or for your convenience you may print this form by clicking below on the appropriate age link. Please complete in blue or black ink and don't forget to bring this form to your child's appointment!
